Please bring the latest medical checkup report (PDF version: printed with your name and date of birth) and visit a medical institution.

If you are visiting a medical institution other than the Health Center or have a primary doctor, fill in the necessary information on the "Report Form" below and send it back to the Health Management Office of each district health center by on-campus mail.

If you have any comments such as "abnormalities in lipids" or "suspected diabetes", come without eating as you may need to have your blood drawn.
If you need treatment for chronic diseases such as dyslipidemia and hypertension, you will be referred to an off campus clinic since medication for such diseases are not available at the Health Service Center.
Bring your checkup report and patient ID card if you have one.
