Check-ups for students enrolled in April and in the first semester of the Junior Division

Almost 100% of new students (with the exception of one or two students who are unable to take the health check-up due to special circumstances) take the health check-up every year, which is required by law. If you are unable to take the health check-up due to illness during the new student health check-up period, please contact the Komaba Health Service Center (see the bottom of this page for contact info).

Please keep in mind that the schedule and contents may change without notice.
Please check the website before turning up for your health check-up.

■ Date and Venue

Date : March 27, 28, 29, 30, 31, April 2

*If you cannot find the “合格整理番号” in the schedule, please contact the Komaba Health Service Center.

*Please enter the necessary information using the URL below by 12:00 on March 24, 2023 only if you are unable to take the health check-up on the designated date and time due to reasons such as moving to Tokyo, moving into the dormitory, university procedures, or participating in undergraduate guidances. Please note that you may be asked to provide proof of schedule changes. If you are unavoidably unable to make the designated time, you do not need to submit the form if you can visit to Venue by 11:00 a.m. for the morning students and by 3:00 p.m. for the afternoon students on the same day.
Notice: If you wish to take a health check-up on a different date than the one specified, please allow plenty of time for the check-up.

*Those who need special consideration, such as difficulty in taking a group health  check-up, should contact the Komaba Health Service Center by 16:00 on March 22, 2023.

■Online Medical Questionnaire

Be sure to complete the Online Medical Questionnaires prior to the health check-up as the Online Medical Questionnaires is also part of the health check-up. Unless you answer the Online Medical Questionnaires, your health check-up will not be completed.
A UTokyo Account is required to conduct the online Medical Questionnaires. For more information, please check the notification from your department.

Online Medical Questionnaires will open 5:00 pm on March 22.

Special Care Required when Attending University 【就学に関する情報提供書】(Applicable persons only)

At the University of Tokyo, 105 minutes of physical exercise / health science training (physical education practice) is compulsory once a week. In order to ensure the safety of students life in their daily lives, including this training, and to start their university life without delay, please make sure to fill in and submit the 【就学に関する情報提供書】 to the Komaba Health Service Center if you need special consideration or restrictions regarding university attendance.
