Persons required to take this health check-up are as follows:
New and re-registered staff handling radioisotopes/radiation.
Those deemed necessary to undergo the health check-up based on the regular RI questionnaire

*1: Must be registered in UTRadMS.
*2: Must have answered the regular RI questionnaire from the above system and have been determined as "subject to health check-up".

*If you do not bring the Check-up sheet with you, you will not be able to take the check-up.
Please note the following points on the Health Check-up:
‣ Check-up sheet :
・Check-up sheet needs to be prepared on UTRadMS, which can be accessed from UT-net or UTokyo VPN.
・Please read the notes carefully and make sure to print out so that it fits an A4 size paper. It is important that the barcode in the upper left corner of the Check-up sheet is readable and that it can record details of the medical examinations. Bringing a screen display or inappropriate Check-up sheet will not allow you to receive the check-up.
・If you are unable to print, please contact the person in charge of your department in advance. Links are in Japanese only.
‣ blood test :
・Eating and drinking does not affect the blood test.

Reservation site
Please make sure to book your reservation at least 3 days in advance of the day you wish to have your Health Check-up.
You will not be able to make a reservation after the available period.
*Reservations will begin from late March.
How to access the results
Please use the system to view your health check-up results from September 2022 onwards
Those who requested the unification of common IDs can verify as a faculty or staff member from February 28, 2024.

After checking your results
If an abnormal value was reported in your result, please seek consultation with a doctor.
Please inform the Health Service Center the result of the consultation, using the below response form so that we may be aware of the state of your health.
