Influenza Vaccination, more available now. (Hongo)

Eligibility: Students, faculty and administrative staff of the University (those who possess staff or student ID cards issued by the University of Tokyo).

(We have the exact amount when paying. If change is required, it is possible that you will be required to wait.)
What to bring on the day:
- Completed influenza vaccination form
- Download the form and the example from here, print it double-sided and fill in the necessary information and bring with you on the day.
- Take your temperature and fill in the information on the form.
- Signature of parents or guardians will be required by underaged minors.
- Staff or student ID card issued by the University of Tokyo.
- Vaccination fee: JPY 2,500 for students, JPY 3,000 for staff (cash only and please bring exact change)
Other important information:
- You will receive an interview from a physician on the day of the vaccination. Please note that depending on the diagnosis given by the doctor you may not be given the vaccination.
- Please read the “Key Facts about Influenza (Flu) & Flu Vaccine ” manual carefully before your visit.
- People who experience heavy secondary reactions after vaccinations cannot be inoculated at the health center.
- Please make sure to come promptly. If you are late for your appointment, your reservation will be cancelled.
- Please refrain from making telephone inquiries regarding the vaccination.
- Please assent that the vaccination may be provided first-come-first-served.
