Health check-ups are required by law. Please make sure you attend the check-ups during the specified period.
Almost 100% of new students (with the exception of one or two students who are unable to take the health check-up due to special circumstances) take the health check-up every year, which is required by law. If you are unable to take the health check-up due to illness during the new student health check-up period, please contact the Hongo Health Service Center (see the bottom of this page for contact info).
1. Date and Venue

＊Please check your “合格通知書” for your “合格者整理番号”

2.Online Medical Questionnaire
＊Please make sure to complete it by Sunday, March 29 (2026).
Be sure to complete the Online Medical Questionnaires prior to the health check-up as the Online Medical Questionnaires is also part of the health check-up. Unless you answer the Online Medical Questionnaires, your health check-up will not be completed.
Online Medical Questionnaires will open on March 23, 2026.

3. Items Necessary to Be Brought to the Health Check-up
① Student ID Card or UTAS(UTokyo Academic affairs System) “Student Info” screen (if you have not yet received your Student ID Card).
② Please wear or bring a plain T-shirt.

④ Plastic bag (A bag that you can put your shoes in and carry around)

5.(Applicable persons only) Submission of 【身体運動・健康科学実習に関する情報提供書（体育用）】
At the University of Tokyo, 105 minutes of physical exercise / health science training (practical physical education) is compulsory once a week.
If you need considerations or restrictions regarding exercises, please make sure to fill in and submit the 【身体運動・健康科学実習に関する情報提供書（体育用）】 to the Komaba Health Service Center by April 9, 2026.

6.Health Check-up Schedule Changes
＊Please cooperate by coming at your assigned time so that everyone may complete their health check-up smoothly.
■When schedule changes are permitted
We accept schedule changes only if you are unable to attend on the designated date for one of the following reasons:
• Circumstances related to moving to Tokyo or entering a dormitory
• Outpatient or inpatient medical treatment
＊Your assigned date and time changes are not allowed for personal reasons such as travel or participation in various orientation events. Please give priority to attending the health check-up.
≪Procedure for Changing Your health check-up schedule≫
You may be asked to submit documents that verify the reason for your assigned date and time change.
1.Start of Application Period: Monday, March 23
2.Please make any changes to your assigned date and time on the website (UTokyo Account required). The website will be available later.
■If you can come on your assigned day but want to change the time:
If you can come at one of the times below, you do not need to submit a request.
- Students assigned to the morning: You may come before 11:00 a.m. on your assigned day.
- Students assigned to the afternoon: You may come before 3:00 p.m. on your assigned day, during a time slot for your gender.
＊Please note that on Tuesday, March 31 and Friday, April 3, there is a gender changeover partway through the afternoon session.
