Notice regarding the handling of personal information by the Division for Health Service Promotion

The University of Tokyo Division for Health Service Promotion  (hereinafter referred to as the Health Service Center) is engaged in maintaining and improving the health of students and faculty members through health management and medical care. This process produces records of your health care status and the diagnosis and treatment of your illness, which contains a lot of your personal information.

Regarding the handling of personal information at the Health Service Center, we abide by the various laws and governmental regulations, "Rules for Disclosure of Personal Information of the University of Tokyo" (東京大学個人情報開示等に関する規則), "Rules for Measures for Appropriate Management of Personal Information Held by the University of Tokyo"(東京大学の保有個人情報の適切な管理のための措置に関する規則), "Internal Regulations on Measures for Appropriate Management of Personal Information Held by the Division for Health Service Promotion" (東京大学保健・健康推進本部の保有する個人情報の適切な管理のための措置に関する内規), and we consider the protection of your privacy to be the most important matter and pay close attention to its handling.

We, the Health Service Center, actively disseminate information obtained in the process of health management activities if such information is considered to be valuable for promoting the the public health. We believe that is a part of our the responsibility as medical professionals and the improvement of the health management situation through them is directly and indirectly beneficial for improving your health. We ask for the understanding and cooperation of students and faculty members regarding this matter.

Therefore, we explain here the handling of the records that contain your personal information at the Health Service Center as follows. We hope that you will understand the role of the Health Service Center as a health management / medical institution and as a health management education and research institute.


■ 1.About the purpose of using personal information

Personal information of students, faculty and staff (hereinafter referred to as "clients") who use the Health Service Center will be used for the purposes listed in the appendix. Please refer to the appendix for details.

■ 2.When we use your personal information for purposes other than those listed in the appendix, you will be asked to give your written consent. This consent may be withdrawn in writing without any disadvantage.
■ 3.About your rights


The rights of clients to personal information managed and operated by the health center are as follows. For disclosure requests based on the "Act on the Protection of Personal Information Held by Independent Administrative Institutions, etc." (「独立行政法人等の保有する個人情報の保護に関する法律」) and  the University of Tokyo Rules for Disclosure of Personal Information follow the rules regarding such matters.

I. Disclosure of personal information

  1. Clients can request disclosure of their personal information. Please note that we may not always be able to respond to disclosure.
  2. Please contact the person in charge below for details regarding the request for disclosure. For information on your own medical treatment, please consult your doctor.

II. Right to request correction of personal information

  1. Clients can request correction of their personal information within 90 days from the date of receiving the disclosure of their personal information. Please note that we may not always be able to respond to this request for correction.
  2. Please contact the person in charge below for details regarding the request for correction.

III. Requests for suspension of use of personal information

  1. Clients can request the suspension of use, deletion and provision of personal information within 90 days from the date of receiving the disclosure of his / her personal information for any of the following reasons. Please note that we may not always be able to respond to this request for suspension of use.

A. The personal information is handled beyond the scope necessary to achieve the purpose of use of the health center, or the personal information was obtained by deception or other fraudulent means, the personal information will be displayed.
B. The personal information is provided to a third party without the prior consent of the person, beyond the scope stipulated by law.
