By completing and submitting the "Things required for substitution" in the table below, you will be deemed to have taken the health check-up conducted by the university.

*If you are cannot register your medical questionnaire, please contact the Hongo Health Service Center (be sure to provide your common ID, student ID number/staff number, and name).

*(For faculty and staff) If you would like the Health Center to manage non-statutory health check test items conducted by the Health Center, please answer the question in the web interview, “I would like the health center to manage the test results I submitted” with “I would like to have an examination”. (There is no need to sign on the front of the distributed consultation slip.)

<Notes on submitted documents>

*Only Japanese or English documents accepted

*Results must be submitted on paper, with copies of all pages without excerpts (data such as film or CD-R cannot be accepted).

*Chest X-rays and electrocardiograms must have been evaluated by a doctor (a doctor's evaluation refers to a judgment such as “no abnormality”, “reexamination in 1 year”, or “requires detailed examination”). For other items, only those that include specific numerical data, judgments, and findings are acceptable.

*If any items are missing, please also submit the results of a separate test at an external medical institution. (Additional tests other than chest X-rays can be performed at the Health Center's Internal Medicine Department.)

 *Those who underwent a medical examination within the applicable year (for example, to substitute for the medical examination in 2024, results from the examination date between April 1, 2024 and March 31, 2025 are required)

               Note) However, if you wish to make use of the substitution procedure for the health check-up at the time of employment for faculty and staff of this university, the result must be from a health check-up that was taken within three months before or after your start of employment.

               Note)However, if you wish to make use of the substitution procedures for the health check-ups for overseas dispatched workers for faculty and staff of our university, the result must be from a health check-up that was taken within 6 months before the date of departure overseas and within 2 months of returning to Japan.

*The name of the medical institution where you received treatment is written on the document.

*Hearing test results are “1000Hz 30db, 4000Hz 25db”

*Please clearly state whether or not you can participate in physical education classes.

If there are any abnormal values or abnormal findings in your medical examination results, please follow the instructions of the medical institution where you visited. Please follow any instructions regarding lifestyle habits (including smoking, drinking, etc.). Only items that are included in the university's health check-up will be processed as substitutes at the Health Center.
