Using health check-up results from external medical institutions as a substitute.

By completing and submitting the "Things required for substitution" in the table below, you will be deemed to have taken the health check-up conducted by the university.

<What you need for a substitute> 

  • ① Take the web interview    Go To Questionnaire
  • ② Submit a medical certificate, etc. (a copy of the health check-up result notification is acceptable) in which you have satisfied all the test items of the applicable health checkup.
  •  *There are precautions regarding substitutions for Check-ups for students enrolled in April and PEAK students.
  •  ・Please clearly state whether or not you can participate in physical education classes.
  •  ・You can also use this format (Japanese)
  • Submit a copy of your Student ID card

<Where to submit>

Submit directly to the Hongo Health Service Center
 *Students enrolled in April can also submit to the Komaba Health Service Center.
Submit electronically
  ・Send an e-mail to the Hongo Health Service Center. We will inform you where to submit the certificate.
  ・The subject line of your e-mail should be “Request for Substitute Student health check-up”.
Please include your name and common ID number in your email text.
Mail Submission
  ・We recommend that you send results using a method that is trackable, such as Letter Pack or registered mail.
  ・The Health Service Center is not responsible for lost packages.

<Submission Deadline>

  • Within the applicable year
  • *Check-ups for students enrolled in April ends on the second Friday of April.

 ・Please answer this questionnaire after your health check-up slip has been distributed.
 ・For those taking the Health Check-up for Newly Hired Employees or the Health Check-up for Long term Travelers, please answer the Web Questionnaires after you are contacted by the person in your department.

②Submit a medical certificate, etc. (a copy of the health check-up result notification is acceptable) in which you have satisfied all the test items of the applicable health checkup.

 ・Only items included in the university health check-up will be treated as substitutes. 
 ・There are precautions regarding substitutions for Check-ups for Newly Hired Employees.
 *Hearing test results are “1000Hz 30db, 4000Hz 30db”

<Submission Deadline>

For regular health check-ups, until the end of February within the applicable fiscal year.

3. Common Items

*Please do not make a health check-up appointment.
*If you are cannot register your medical questionnaire, please contact the Hongo Health Service Center (be sure to provide your common ID, student ID number/staff number, and name).

<Notes on submitted documents>

*Details of health check-up items can be found here.
*Only Japanese or English documents accepted
*The name of the medical institution where you received treatment is written on the document.
*Those who underwent a medical examination within the applicable year (for example, to substitute for the medical examination in 2025, results from the examination date between April 1, 2025 and March 31, 2026 are required)
*However, if you wish to make use of the substitution procedure for the health check-up at the time of employment for faculty and staff of this university, the result must be from a health check-up that was taken within three months before or after your start of employment.
*However, if you wish to make use of the substitution procedures for the health check-ups for overseas dispatched workers for faculty and staff of our university, the result must be from a health check-up that was taken within 6 months before the date of departure overseas and within 2 months of returning to Japan.
*Results must be submitted on paper, with copies of all pages without excerpts (data such as film or CD-R cannot be accepted).
*Chest X-rays and electrocardiograms must have been evaluated by a doctor (a doctor's evaluation refers to a judgment such as “no abnormality”, “reexamination in 1 year”, or “requires detailed examination”). For other items, only those that include specific numerical data, judgments, and findings are acceptable.
If any items are missing, please also submit the results of a separate test at an external medical institution. (Additional tests other than chest X-rays can be performed at the Health Center's Internal Medicine Department.)

If there are any abnormal values or abnormal findings in your medical examination results, please follow the instructions of the medical institution where you visited. Please follow any instructions regarding lifestyle habits (including smoking, drinking, etc.). Only items that are included in the university's health check-up will be processed as substitutes at the Health Center.

◇The substitute results can be confirmed through the health check-up reservation system. The timing the results are released may vary depending on the contents of the health check-up. Please be sure to check the released results.
