・Official Notice of Your Student ID Number or Student ID Card (In case you have not recieved a Student ID Card yet, please bring your Official Notice of Your Student ID Number.)

*For those who wish to receive assistance with medical examinations
● Medical Information Form for physical exercise
At the University of Tokyo, 105 minutes of physical exercise / health science training (practical physical education) is compulsory once a week.
If you need considerations or restrictions regarding exercises, please make sure to fill in and submit the “Medical information Form for physical exercise” to the Komaba Health Service Center by September 26, 2025. (Weekdays only, 9:00～16:30)

A few "General slots" have been set aside for those who have difficulty in taking health check-ups as a large group, those who can only take a health check-up on a specific day, those who wish to be able to change alone, or those who are injured and have difficulty in moving around at the pace of others.
This “General slot” is open to all, regardless of gender, and the time required for the health check-up will be longer.
Due to the limited number of slots available, we ask for your understanding and cooperation so that those who are in absolute need of such considerations can choose this slot.
The operation of this slot may be changed or cancelled in the future.
