Influenza vaccination on all three campuses will be done through reservations only.
The start time and the button enter the website to make reservations differ for each venue.
The "Go to reservations" button can be found at the bottom of this page.
Influenza vaccines are delivered in several batches. The schedule and reservation slots will be increased from time to time depending on the storage status of influenza vaccines. Please check our website regularly.
We can prepare a limited number of influenza vaccines, so if you find a clinic that can give you a vaccine, get one wherever available. We recommend that you get a influenza vaccine every season at any place.

Students, staff and faculties of the University of Tokyo (those who possess Student or Staff ID cards issued by the University of Tokyo).

What to bring on the day of the vaccination
*You need to bring 1) - 4) as noted below, otherwise you will not be able to receive the vaccination.
1) Completed influenza vaccination questionnaire
Download the form from here. Print it double-sided and fill in the necessary information and bring it with you on the day of the vaccination.
・Take your temperature on the day you are receiving the vaccination and fill in the information on the form.
・Please fill out influenza vaccination form with black ink that cannot be erased. Do not use pencils.
2) Student or Staff ID card issued by the University of Tokyo.
3) Vaccination fee
*Cash only. Please make sure to have the exact amount ready when paying.
4) Printed reservation information from the website or the email sent automatically on completing the booking.
A print out of the screen, memo or photo image of the screen are acceptable.
*Please cooperate in wearing a mask, preferably made of non-woven fabric.
Other important information
・You will receive an interview from a physician on the day of the vaccination. Please note that depending on their diagnosis, you may not be given a vaccination.
・Please carefully read the “Key Facts About Seasonal Flu Vaccine” manual before your visit.
・People who experience heavy secondary reactions after vaccinations cannot be inoculated at the Health Service Center.
・Please make sure to come on time.
・Please be sure to check the venue on our website before inoculation.
Reservation
・Please select the venue you wish to attend and your status (student/staff and faculty) on the reservation site and make a reservation. Bookings made by staff on a students’ schedule will be voided (and vice versa.) Please be careful not to make a mistake.
・Authentication with your UTokyo account is required to access the reservation site.
・[Common ID and (students only) Student ID number] are required when making a reservation. (Common ID is an 18-digit number printed on the bottom right of your student or staff ID card. ⇒ Click here for explanation about the Common ID.)
・We do not accept reservations by phone.
・You can confirm, amend or cancel your reservation from the reservation completion email.
※Please note that the reservation start date and time are different for each venue.
